    Summer Training 2019 Registration Form

    If you are new to OAFC and would like to attend Summer Training, please submit a letter of recommendation for maturity and desire to participate from your pastor or local OAFC group leader to [email protected]. 
    If you will be driving youth to Summer Training, please make sure you have every person's Medical Release Form fully filled out and with you before you leave - for your liability and for OAFC's. You can download a copy of the Medical Release Form at the top of this page.

Please download and submit this Medical Release form and Model Release for this year's Summer Training, either by emailing it to [email protected] or bringing it with you to Training. Thank you.

After Submitting your completed Registration, please pay for Registration either by paying online, sending in a check to the OAFC Business Office, or paying at the registration table. Registration fees are $50 per attendee, not to exceed $200 per family.
